STEP 2: Attend your scheduled Virtual Brief
Meeting will be held on Microsoft Teams.
Link will be emailed after confirmation.
STEP 3: Submit Applicant Documents
- Completed Application (PDF fillable)
- Admissions Packet (PDF fillable)
- Medical Forms
- Mentor Application (PDF fillable)
- Family Tree Online Form
- Government Issued ID (State ID, Driver’s License, Military ID, or Passport)
- Birth Certificate
- Social Security Card
- I-94 Card, Permanent Resident card or equivalent for Non-US Citizens
- Medical Insurance Card

- Acceptance Letter (All potential candidates will be notified of acceptance and invited to the Scholarship Award Ceremony)
PHASE 4: Attend Scholarship Award Ceremony & Final Endurance by INVITE ONLY
